Application Envelope Checklist

 

To expedite the processing of your application, please PRINT the following information on the back of the envelope in which you mail your application:

  • Last / family name
  • First name
  • Academic department
  • Semester and year to which you are applying
  • either ONLINE APPLICANT or APPLICATION ENCLOSED

    Checklist of potential enclosures:

  • Application fee, $60 (if not submitted online) 
  • Official (sealed) transcripts from every college / university attended
  • Resume (if not submitted online)
  • 3 sealed references, with signature of reference writer across sealed flap
  • Personal statement (if not submitted online)
  • CBEST / PRAXIS / CSET scores (if applicable)
  • Other items as required by the academic program to you are applying

    Note:  Official GRE, GMAT, and TOEFL scores must be sent directly from the testing agency to CGU. Click here for additional information about test scores.
