Mathematica for Students Information and Check-out Form
Claremont colleges have license for the latest version of Mathematica for faculty and students. To check out the installation CD from SMS office, please fill in the form and turn it in to Jeho or Susan by the time you check the installation media out. The period of check-out is one week. Please return it back to SMS office within a week(1).
Qualification: A student who installs "Mathematica for Students" on his/her personal computer must be a current/active student at CGU or other Claremont colleges (-- Joint PhD students must be registered either at CGU, San Diego State University, or Long Beach State University).
Installation: Follow the authorization instruction included in CD case.
Restrictions: You must return the CD after installing it on your computer. You are not allowed to copy the CD. If you need to re-install the Mathematica, please request the installation CD from SMS.

(1) For students in Long Beach State and San Diego State joint Ph.D. programs, the CD may be handed in to another student without being returned to office for convenience. (S)he must also sign the form and turn it in to Jeho or SMS office before (s)he gets the CD from the previous possessor. As stated above, no one should keep it for more than a week.
