• One-page cover sheet listing the title of the work and the poet's name, address, telephone number, fax number, e-mail address, if any.
• List of previously published work.
• One completed copy of the entry form (printed)

Entries must be postmarked no later than September 15, 2007, to be eligible. The winners of the 2008 Kingsley Tufts Poetry Award and Kate Tufts Discovery Award will be announced in February, and the awards will be presented in April 2008.
